Both companies are led by Surya Pal Chahar, CEO, and both were brand-new at the time of engagement, with no credentialing history of any kind.

The Challenge

When Surya Chahar engaged CareLumi in early April 2026, neither entity had any credentialing infrastructure. There were no verified provider files, no payer enrollments, no DataSpring (formerly CAQH, the Council for Affordable Quality Healthcare, which maintains a centralized database used by payers to collect and store provider credentialing data) profiles, no federal exclusion monitoring, and no internal process for managing any of it.

The consequences were immediate and structurally linked. Without PSV reports, hospital administrators cannot grant privileging (the process by which a healthcare organization formally authorizes a provider to perform specific clinical procedures based on verified training and competency). Without privileging, Frado.Ai's pathologists could not be placed and could not generate revenue.

On the facility side, Frado Healthcare LLC could not bill Medicare or MO HealthNet without enrollment. The sequence was also legally mandatory: MO HealthNet's Type 70 enrollment cannot be submitted until a Medicare Provider Transaction Access Number (PTAN) is issued, meaning every upstream delay compounds the timeline directly.

Surya evaluated traditional credentialing organizations before CareLumi, including one offshore firm and several U.S.-based companies. The alternatives quoted approximately five weeks for verification reports alone. As a startup CEO managing a lean team with no dedicated credentialing staff, Surya needed a partner who already knew what needed to be done and could move fast without requiring him to manage the process.


Why They Chose CareLumi

Frado's business model requires onboarding a large number of pathologists across multiple hospital clients, each operating under different privileging standards and insurance requirements. Surya needed a partner capable of handling both provider verification and facility enrollment under one engagement, at the scale Frado's pipeline demands.

Implementation

CareLumi launched two parallel workstreams on the day the contract was signed, with both tracks active from engagement initiation.

Frado.Ai: Pathologist Verification

CareLumi built the full intake infrastructure from scratch, including provider intake forms using a single-use, 72-hour magic link (so pathologists never need to log into a portal), a PSV tracker, and a DataSpring onboarding workflow. The Pathologist Verification Standard Operating Procedure version 1.1 was issued in the weeks following engagement start.

A growing pipeline of pathologists completed intake and received uploaded verification reports. Each report covered state licenses, board certifications, Drug Enforcement Administration (DEA) registration, Educational Commission for Foreign Medical Graduates (ECFMG) status where applicable, post-graduate training, and malpractice history, all verified against primary sources.

Federal exclusion monitoring is now live on a 30-day cycle for all providers, covering the Office of Inspector General (OIG) List of Excluded Individuals and Entities (LEIE), System for Award Management (SAM.gov), Office of Foreign Assets Control (OFAC), the Centers for Medicare and Medicaid Services (CMS, the federal agency that administers Medicare and Medicaid payment policy) Preclusion List, Medicare Order and Referring file, CMS Opt-Out, and state Medicaid exclusion lists per licensed state per provider.

Frado Healthcare LLC: Facility Enrollment

CareLumi executed a six-phase build for the Medicare enrollment process. Phase 0 locked the canonical entity identity, assembling the core document package from state registration records, IRS entity confirmation, the Clinical Laboratory Improvement Amendments (CLIA, the federal regulatory framework governing laboratory testing quality standards) certificate, and Commission on Office Laboratory Accreditation (COLA, the accrediting body for independent clinical laboratories) correspondence.

Phase 1 covered COLA accreditation and lab director verification, including a full re-credentialing (the periodic process by which providers renew their verified credentials with payers and health systems) cycle following a lab director transition. Phase 2 locked all organizational data for submissions and confirmed bank verification. Phase 3 secured the Type 2 National Provider Identifier (NPI) in the National Plan and Provider Enumeration System (NPPES). Phase 4 built the Centers for Medicare and Medicaid Services 855B form (CMS-855B, the facility enrollment application required for independent clinical laboratories to participate in Medicare). Phase 5 covers the MO HealthNet application, pending Phase 4 completion. Phase 6 covers post-enrollment compliance.

Multiple compliance obstacles were identified and resolved across the engagement, with no CEO involvement required in remediation. These included a facility address discrepancy across regulatory documents, a lab director transition requiring a full primary source verification cycle to federal standards, a CLIA certificate type issue resolved through the COLA accreditation process, a National Practitioner Data Bank (NPDB) peer review document rejection resolved by drafting a standalone document to the 45 CFR 60.3 standard, and a missing baseline enrollment history resolved through a formal Provider Enrollment, Chain and Ownership System (PECOS) negative search.

By the time the engagement reached its completion phase, the Type 2 NPI was confirmed in NPPES, documentation was uploaded and organized across all required categories in the CareLumi Credentials Verification Organization (CVO, a platform or body that manages the full credentialing and compliance lifecycle for healthcare providers) platform, and the CMS-855B packet was substantially complete. Submission is pending two external approvals outside any credentialing vendor's control: the CLIA Certificate of Accreditation, awaiting CMS issuance following COLA survey completion, and CMS Identity and Access Management surrogate approval.
